Thank you for joining the Periphex market study for NoxyPure.  After each of your 10 treatments, we request that you take the corresponding survey for each application. Just select the appropriate survey link below to begin. 

Each survey takes approximately 3 to 5 minutes.  Please remember to complete it after your treatment.  It is very important that you complete your 10 applications and answer each short survey (unless directed to stop).
